Join Living Well Nutrition Membership

LIVING WELL NUTRITION & SUPPLEMENT MEMBERSHIP AGREEMENT

By enrolling in the Join Living Well Nutrition & Supplement Membership Program, you agree to the following terms:

MEMBERSHIP & BILLING
Your Transformation membership is billed at $159 per month. Your first payment is due at enrollment. Subsequent payments are automatically charged on the same day each month to the payment method on file. You authorize Join Living Well Nutrition to charge your payment method on a recurring monthly basis until you cancel.

CANCELLATION
You may cancel your membership at any time by contacting our team. Cancellations take effect at the end of the current billing period. No partial-month refunds are issued.

SUPPLEMENT AUTO-SHIP
As a member, you will receive automatic monthly shipments of your recommended supplements. You may adjust, pause, or cancel your auto-ship order by contacting your practitioner at least 5 business days before your next scheduled shipment.

MEMBER PRICING
As much as 35% off from manufacturer's online prices applies to all supplement orders placed through your membership account. Pricing is subject to change with 30 days' notice.

PRACTITIONER SERVICES
Practitioner services included with your plan are provided as described in your membership tier. Priority text access is available during normal business hours. Phone/Zoom consultations (Elite plan) must be scheduled in advance.

PRIVACY
Your personal and health information is kept strictly confidential and is used solely to provide your membership services. We do not sell or share your information with third parties.
